I agree to the terms and conditions. I understand that the live audience portion of the EntreMD Live 2024 event is for physicians only and certify that I am a physician. I am granting permission for EntreMD to run a one-time payment of $597 on my credit card for the EntreMD Live 2024 conference. I understand this is non-refundable. By agreeing to these terms and conditions, I acknowledge that there are NO cancellations or refunds regardless of use or participation, NO exceptions. I understand this payment does not include accommodations or airfare.

Media Release Statement:
The EntreMD Live organizers will be taking photos & videos during the conference for promotional use. I grant and authorize the organizers the right to take, edit, alter, copy, exhibit, publish, distribute and make use of any and all pictures or video taken of me to be used in and/or for legally promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, websites, social networking sites, and other print and digital communications, without payment or any other consideration. This authorization extends to all languages, media, formats, and markets. This authorization shall continue indefinitely unless I revoke said authorization in writing. I understand that if I select do not want my my image in any photos or videos, I will inform the organizers in writing at druna@entremd.com. Depending on the size of the picture or group picture, I may still be in the background of a photo/video used for promotional use.
